I just got mine this morning - I've had toons parked on 4 different servers trying to find a Minfernal spawn. This morning I had some time to sit and wait so I killed sentries for about 10 minutes and had one spawn on top of a corpse pile. I know it could be total coincidence but considering the flavour text blizz added to this item "These ill-tempered constructs of flame and rage sometimes rise from the remains of larger infernals who fall in battle" I could be right!
